Cortisol, Stress, and Olive Oil: What the Dietary Research Has Found

When chronic stress disrupts cortisol output, the overall dietary pattern — not any single food — appears to matter most. Here is a look at what current research says about the Mediterranean diet, the HPA axis, and where extra-virgin olive oil fits in.

Stress, Dietary Fat, and the Connection Between Them

For most people today, stress is close to a constant — triggered by work pressures, caregiving demands, and relentless digital overload. At the centre of the body's stress response sits cortisol, a hormone produced by the adrenal glands. In the short term, cortisol is essential: it raises blood glucose, modulates immune activity, and helps the body mobilise in a crisis. When cortisol output remains chronically elevated, however, it has been reported to be associated with sleep disruption, difficulty regulating body weight, and shifts in inflammatory markers.

What has begun to attract sustained attention in nutritional neuroscience is the possibility that what we eat can influence how the HPA axis — the hypothalamic-pituitary-adrenal pathway — responds to stress. In particular, researchers have been exploring the hypothesis that the type of dietary fat consumed, whether predominantly saturated or predominantly monounsaturated (MUFA), may engage inflammatory signalling pathways and stress-hormone regulation mechanisms differently. This remains an active area of inquiry rather than settled science.

The Mediterranean Diet and the HPA Axis — What Research Reports

The Mediterranean dietary pattern places extra-virgin olive oil at the centre of fat intake and builds around vegetables, whole grains, fish, and nuts. Several studies have examined how closely following this pattern relates to stress response.

A systematic review published in Nutrients in 2021 found that populations with higher adherence to the Mediterranean dietary pattern were associated with lower levels of psychological stress indicators and inflammatory biomarkers compared with those with lower adherence. The researchers explicitly noted that these findings do not establish causation and that confounding variables related to broader lifestyle factors are difficult to rule out.

Separate analyses drawing on data from the PREDIMED cohort study, conducted in Spain, reported that regular consumption of extra-virgin olive oil (EVOO) was significantly associated with changes in oxidative stress markers and pro-inflammatory cytokine levels. The researchers proposed that polyphenol compounds in olive oil — including oleocanthal and oleuropein — may be important mediating factors in that association, while adding that it would be premature to attribute the effect to any single component.

Polyphenols and Stress Signalling Pathways

Among the bioactive constituents of EVOO, polyphenols attract the most research interest. According to International Olive Council (IOC) standards, polyphenol content in EVOO varies considerably depending on cultivar, harvest timing, and pressing method, with measured values generally ranging from several hundred to over 1,000 mg/kg.

Polyphenols are reported to contribute to the modulation of the NF-κB pathway — nuclear factor kappa-light-chain-enhancer of activated B cells — which is a central regulator of inflammatory signalling. NF-κB has been reported to interact with cortisol under conditions of chronic stress, forming inflammatory feedback loops. A 2022 study published in Frontiers in Nutrition observed that adults who regularly consumed high-polyphenol olive oil tended to show lower measured levels of 8-isoprostane, one marker of oxidative stress. As with the other studies cited here, this research was largely correlational in nature, and further investigation is needed before causal claims can be made.

How to Use Olive Oil Within a Broader Dietary Pattern

A point researchers consistently emphasise is that the relevant variable is not olive oil in isolation but its role within the overall dietary pattern. Adding olive oil to a diet built primarily around ultra-processed foods high in saturated and trans fats does not replicate the context in which the research associations were observed.

In the Mediterranean patterns examined in the literature, olive oil is used across both cooking and finishing applications at roughly 20–40 ml per day. To limit polyphenol loss from heat, food scientists generally recommend using EVOO as a dressing or as a finishing drizzle over completed dishes. At the same time, multiple food science sources confirm that EVOO is suitable for light sautéing within its smoke-point range, provided temperatures are kept moderate.

On the question of quality, choosing oils that display the harvest year, acidity percentage, and polyphenol content on the label provides useful information. Under IOC and EU regulations, extra-virgin grade olive oil must have free fatty acid content (acidity) of no more than 0.8%. Oils with lower acidity are generally understood to reflect fresher fruit and better polyphenol preservation.

The Limits of a Dietary Approach to Stress

The growing body of research suggesting that dietary patterns may contribute to stress hormone modulation is genuinely interesting. Yet cortisol regulation is shaped by multiple interacting factors: sleep quality, physical activity, social support, and psychological resources all play roles. It is too early to conclude that nutritional intervention alone can govern the entire stress response, and the academic consensus is clear that a large proportion of the existing research consists of observational studies or small-scale clinical trials — meaning that confirming the direction of causality will require substantially more randomised controlled trials.

The Mediterranean dietary pattern is presented as one model of balanced eating by a range of credible institutions, and olive oil continues to be among its most actively studied components. The idea that choices made at the table might serve as a modest, accessible starting point in an otherwise demanding daily life remains a question worth continuing to explore.
